چکیده

Drug-induced pulmonary arterial hypertension (PAH) is constantly evolving as new drugs are developed. Carfilzomib a recently approved therapy for relapsed and refractory multiple myeloma. While it has been associated with cardiovascular adverse events, such ischemic heart disease failure, PAH not well-described side effect. We present two patients who developed initiation of carfilzomib. They both initially presented severe dyspnea, had elevated right ventricular systolic pressure on transthoracic echocardiography ultimately underwent catheterization. With discontinuation carfilzomib, improvement in hemodynamics. However, one patient required PAH-targeted therapies worsening function again despite permanent It important to recognize the association between carfilzomib PAH. Echocardiography can be an initial screening tool. from may reversible, especially if diagnosed early; however, extended follow-up essential.

Pulmonary hypertension and multiple myeloma

The relationships between pulmonary hypertension multiple myeloma, monoclonal protein and finally drug treatment of plasma cell diseases are complex and not fully elucidated. Pulmonary hypertension has been reported as a rare complication of multiple myeloma (MM), more frequently in patients with POEMS syndrome. More recently pulmonary hypertension has been described during thalidomide treatmen...

THE EFFECTS OF CAPTOPRIL ON PULMONARY AND SYS TEMIC ARTERIAL PRESSURES IN HIGHALTITUDE PULMONARY HYPERTENSION

The purpose of this investigation was to assess the effect of captopril on both systemic (P.a) and pulmonary arterial pressures (PPA) in patients with high-altitude pulmonary hypertension (HAPH). Seventeen patients (mean age 44±6.8 years) with HAPH and mild to moderate systemic arterial hypertension were included in the study. All patients underwent right heart catheterization with measurem...

Pulmonary hypertension related to thalidomide therapy in refractory multiple myeloma.

Thalidomide is an effective treatment both in advanced multiple myeloma (MM) and as first-line therapy in combination with dexamethasone or other cytotoxic chemotherapy [1, 2]. Two cases [3, 4] of pulmonary hypertension in the absence of thromboembolic events have been described in patients with MM during thalidomide treatment [5]. A 63-year-old woman was diagnosed in 1998 with IIA MM IgG kappa...

The Importance of Electrocardiography in Pediatric Patients with Pulmonary Arterial Hypertension in Follow- up

Background: Right Ventricular (RV) hypertrophy is an adaptive response to chronic RV pressure overload in patients with pulmonary hypertension. We investigated the relationships between RV hypertrophy indicators, including electrocardiography, the percentage oxygen saturation (SaO2%), body mass index (BMI), and blood uric acid levels in patients with...

Reversible Pulmonary Hypertension Related to Thalidomide Treatment for Multiple Myeloma

Multiple myeloma (MM) is thrombogenic as a consequence of multiple hemostatic effects. Thalidomide is an effective treatment; however, it has been associated with an increased risk of thromboembolic events including pulmonary hypertension (PH). PH in the absence of thromboembolic events has also been described in some patients with MM during thalidomide treatment. We experienced occurrence of P...
